is/her name, and business and home address; and if the application is made <br />on bell If #of a art•• hip, association or corporation, the name and address of such partnership, <br />assooiat on, and the phone number of the contact person; <br />(b) If the proposed licensee is a partnership, the name and home addresses of all <br />partners; or if the proposed licensee is an association or corporation, the names and home <br />addresses of its principal officers and managers and of the owners (not to exceed 10) or the <br />largest shareholders of the business or enterprise; and <br />(e) Such information as is required to be furnished by ordinance or is reasonably <br />required by the City Clerk or other applicable City departments. <br />(2) The application shall include documentation indicating insurance coverage, which <br />shall remain in effect during the license term, naming of the City as an additional insured, and <br />non -cancellation provisions, which provide a minimum of 30 days' notice to the City prior to <br />cancellation, as follows: <br />(a) Comprehensive general liability insurance not less than $100,000 for injuries <br />including accidental death to any one person, and subject to the same limit for each person in an <br />amount of not less than $300,000 on account of any one accident; <br />(b) Property damage insurance not less that. $50,000 for each accident and not <br />less than $100,000 aggregated; and <br />(c) Workers compensation insurance coverage of employees as required by state <br />law. <br />(B) Signatures required. The proposed licensee shall sign the application; or if the proposed <br />licensee is a partnership, an association or corporation, at least one person having power under <br />its bylaws to execute contracts of the association or corporation shall sign. <br />(C) Issuance of license. The license shall be granted by the City Clerk. <br />(D) Denial of license; appeal. In the event the City Clerk determines that the application does <br />not comply with the ordinance criteria, the report and recommendation of the City Clerk and <br />applicable staff shall be referred to the City Council, together with the reason or reasons for the <br />proposed denial. A copy of the recommendation shall be supplied to the applicant. The applicant <br />may appear before the City Council to respond to the recommendation of denial. The City <br />Council shall make the appropriate findings and either issue or deny the license application. <br />§ 116.04 STANDARDS FOR LICENSE ISSUANCE; SUSPENSION OR REVOCATION <br />OF LICENSE; TERM. <br />(A) Standards for issuance generally; term. Licenses and renewals thereof shall be issued <br />after a verification of the applicant's qualification and record in the performance and operation of <br />the types of work for which the applicant seeks a license.
