LAKE ELMO SOLAR ENERGY APPLICATION <br />This form is meant to be an addendum to a building permit, interim use permit, conditional use permit, or <br />certificate of zoning compliance application. Please attach to the applicable form to ensure all submittal <br />requirements are met. The following outlines what type of permit is required for each type of solar energy <br />system: <br />SOLAR ENERGY PROJECT INFORMATION <br />1.System Manufacturer: <br />2. Is the system of mirror reflecting design: Yes No <br />4. Please explain how glare will be minimized from affecting adjacent property owners <br />(selective placement, screening, modifying orientation, etc.) <br />_________________________________________________________________________ <br />_________________________________________________________________ <br />Attachments to include: <br /> NPDES permit and/or stormwater management, erosion, and sediment control plan <br />per City and watershed district requirements (only applicable for solar farms). <br /> Interconnection Agreement between the applicant and servicing electrical utility if <br />connected directly to the distribution or transmission system (if applicable). <br /> Site plan/survey showing proposed setback of the system from all property lines, <br />easements, lot size, impervious surface (if ground-mount solar energy system), and <br />structures. <br /> Decommissioning plan including provisions for removal of all structures and <br />foundations, restoration of soil and vegetation, and plan ensuring financial resources <br />will be available to fully decommission the site (may be waived for certain types of <br />solar energy systems). <br /> A statement from an engineer certifying that the foundation and design of the solar <br />panels, racking and support is within accepted professional standards, given local soil <br />and climate conditions (only applicable for solar farms). <br /> Approval from the Minnesota Pollution Control Agency (MNPCA) (if applicable) <br /> System design
