Lake Elmo Medical Center, LLC is currently in the <br />process of expanding further outpatient medical services through securing tenants that also <br />operate within this space including but not limited to: primary care clinic, urology clinic, chronic <br />pain management clinic, urgent care, outpatient laboratory, and high definition medical imaging <br />clinic. <br />d. A specific written description of the proposed use(s). <br />i. Incorporate information describing how the use will work on the <br />proposed site including proposed site changes, existing open <br />spaces, landscaping, traffic circulation, transition areas to adjacent <br />properties, individual uses for existing and proposed structures, and <br />effects on natural areas (wetlands, forests, etc.) both on-site and in <br />the general vicinity of the project. <br />Response: <br />The proposed project consists of an addition to the existing building to support expanded <br />operations. The current structure will remain in place, with the new addition designed to be <br />compatible with the existing development. <br />Site improvements will include the reconfiguration of on-site circulation to better accommodate <br />both passenger and truck traffic. A new drive aisle will be constructed to allow safe and efficient <br />truck access, loading, and maneuvering within the site. Additional parking spaces will be <br />provided to meet current code requirements and support the increased use of the building. <br />Landscaping will be installed and enhanced in accordance with applicable regulations. There will <br />screening where necessary to provide a visual buffer to adjacent properties. Existing open space <br />areas will be maintained where feasible, with improvements focused on aesthetic enhancement <br />and site functionality. <br />The project is designed to integrate with surrounding properties by maintaining appropriate <br />setbacks. Traffic circulation improvements are expected to enhance overall site safety and <br />reduce congestion. Work will not impact the existing wetland. Only the grass/sod lawn area <br />were a building expansion was originally proposed. <br />ii.Provide specific details about the use including the number of <br />employees, hours of operation, maximum number of animals (if <br />applicable), proposed development schedule, etc. <br />Response: <br />The use of the property will expand upon the current outpatient medical offerings that are <br />currently present within the facility (High Pointe Medical Center.) As previously mentioned, Lake <br />Elmo Medical Center, LLC will focus its efforts in securing leases with tenants that provide other <br />supplementary outpatient medical services, as well as house some medical administrative <br />operations. The goal is to be able to provide members of the community with a convenient way <br />to receive high-quality medical care that is also cost effective and person-centric. Hours of <br />operation will be from 7:30AM to 9:00PM; which upholds urgent care hours. All other medical <br />specialty hours of operation will vary, but will be operate within the 7:30AM to 9:00PM <br />timeframe to ensure the most access to needed healthcare services. In order to support all <br />service lines, the amount of employees at the facility will likely fall in the 100-125 FTE range with
