FEW <br />CITY OF LAKE ELMO, MINNESOTA <br />AUTHORIZATION FOR PROFESSIONAL SERVICES <br />TO: Toltz, King, Duvall, Anderson <br />and Associates, Incorporated <br />1500 Piper Jaffray Plaza <br />444 Cedar Street <br />St. Paul, Minnesota 55101 <br />Pursuant to our Agreement dated February 2, 1988, you are hereby authorized to proceed with <br />the professional services described as follows: <br />2006 MSA REPAVING <br />45TH STREET, JULEP AVENUE, AND 47TH STREET <br />I. PROJECT DESCRIPTION <br />In accordance with the Adopted 2005-2009 CIP, the following streets are scheduled to be <br />reclaimed and repaved in 2006: <br />45th Street, Julep Avenue, and 47th Street <br />H. SERVICES TO BE PROVIDED BY TKDA <br />Based on TKDA'S understanding of the Project, we propose to provide the following services: <br />A. PLANS <br />l . Feasibility Report Phase <br />Upon receipt of authorization to proceed with the Feasibility Report Phase, <br />TKDA shall: <br />a. Consult with the OWNER to clarify and define the OWNER'S <br />requirements for the Project and review available data. <br />b. Advise the OWNER as to the necessity of the OWNER'S providing or <br />obtaining from others, data or services of the type(s) described under the <br />- 1 - <br />
