VE <br />CITY OF LAKE ELMO, MINNESOTA <br />AUTHORIZATION FOR PROFESSIONAL SERVICES <br />TO: Toltz, King, Duvall, Anderson <br />and Associates, Incorporated <br />444 Cedar Street, Suite 1500 <br />St. Paul, Minnesota 55101 <br />Pursuant to our Agreement dated February 2, 1988, you are hereby authorized to proceed with <br />the professional services described as follows. Hereinafter, the City of Lake Elmo is referred to <br />as the OWNER. <br />PAVEMENT MANAGEMENT REPORT <br />PROJECT DESCRIPTION <br />This Report will provide a Pavement Management Report including public street inventory and <br />distress assessment for all municipal streets within the City of Lake Elmo. Visual street <br />inspections will include distress type, severity, and measurement. Available data such as street <br />width, pavement thickness, curb type and condition, year built, and average daily traffic (ADT) <br />will be inputted with each street segment. An Overall Condition Index (OCI) will be assigned to <br />each street segment based on the U.S. Army Corps of Engineers Pavement Management <br />Methodology and the Strategic Highway Research Program (SHRP) Distress Identification <br />Manual. This data will be used as the foundation to plan for preventative maintenance and <br />reconstruction projects. The street data will be collected, organized, and stored under TKDA's <br />license of a CarteGraph pavement management software package. Cart8Graph is nationally <br />recognized asset management software. <br />II. SERVICES TO BE PROVIDED BY TKDA <br />TKDA is requested to provide the following services: <br />A. DATA COLLECTION <br />Visual Street Inspection <br />a. Provide street ratings based on USACE and SHRP distresses measured in <br />the field. <br />Review ratings with City staff. <br />
