FISCAL IMPACT: $22,214.00. This change order increases the contract amount for the project by $22,214.00 bringing the revised construction contract to $1,901,647.80. With this change order the project remains within the authorized project budget and contingencies. SUMMARY AND ACTION REQUESTED: The City Council is respectfully requested to consider approving, as part of the consent agenda, Change Order No. 2 for the 39th Street North: Street and Sanitary Sewer Improvements, thereby increasing the
contract amount by $22,214.00 and extending the Contract Times for Substantial and Final Completion. If removed from the consent agenda, the recommended motion for the action is as follows:
“Move to approve Change Order No. 2 for the 39th Street North: Street and Sanitary Sewer
Improvements thereby increasing the contract amount by $22,214.00 and revising the Substantial and Final Completion dates.”

LEGISLATIVE HISTORY/BACKGROUND INFORMATION: This change order is being processed at the request of the City for two scope changes to be completed
with the project. 1. The City requested the placement of geotextile fabric as part of the subgrade preparation for the
road. The fabric was necessary to separate the existing and new subgrade materials due to existing poor soil conditions discovered when the road was excavated. Without the fabric additional subgrade correction would have been required at a greater cost. 2. A manhole had to be core drilled for the placement of an 8" sanitary sewer service stub to accommodate the future development of the adjacent property. This change order also extends the contract times for both Substantial and Final Completion. Substantial Completion has been revised from October 31, 2014 to June 30, 2015. Final Completion is extended from June 5, 2015 to August 15, 2015. These extensions are necessary and justified due the late award of the project (delayed 5 weeks) and early winter weather conditions occurring in the fall of 2014. RECOMMENDATION: Staff is recommending that the City Council consider approving, as part of the Consent Agenda, Change
Order No. 2 for the 39th Street North: Street and Sanitary Sewer Improvements, thereby increasing the contract amount by $22,214.00 and extending the Contract Times for Substantial and Final Completion. If removed from the consent agenda, the recommended motion for the action is as follows:
