WHEREAS, on February 6, 2007, the Lake Elmo City Council reviewed
,�nendment #1 to the September 15, 2005 Professional Services Agreement with Molly
,K Krakowski extending her contract to June 30, 2007 for services on the CDBG
Cimarron Gas Service Project,
NOW, THEREFORE, BE IT RESOLVED, that the City Council approves
,,.Amendment 41 to the September 15, 2005 Professional Services Agreement with Molly
. K.rakowski for her services on the Cimarron Gas Service CDBG Project.

THIS ADDENDUM is made as of the 30th day of January 2007, by and between the City
of Lake Elmo (the City) and. Molly M. Krakowski (the Consultant).
RECITALS
WEEREAS, the City was advised that the Con-ununity Development Block Grant
(CDBG) needs to be administered according to HUD policy; and
WHEREAS, it was determined that the Consultant could provide these services to
administer the above grant; and
WHEREAS, the Consultant and the, City entered into a signed contract on the 15'h day of
S . epttinber 2005 effective through April 2006; and
"A7HEREAS, the Consultant successfully completed all rIe-quirements of said contract.
NOW THEREFORE, in consideration of the mutual undertakings and agreements
hereinafter set forth, the parties hereto mutually agree as follows:
1. The contract end date will be extended through June 30, 2007-01-29
2. Compensation shall be payable to the Consultant for her services provided
hereunder in an amount of $65.00 per hour, not to exceed $12,000,00.
3. All agreements in initial contract remain in place.
IN WITNESS WHEREOF, the parties hereto have caused this Addendum to be executed
as of the day and year first written above.
