RESOLUTION NO. 7542 <br />CITY OF MOUNDS VIEW <br />COUNTY OR RAMSEY <br />STATE OF MINNESOTA <br />APPROVING THERAPEUTIC MASSAGE LICENSE RENEWALS <br />WHEREAS, the City Code requires that massage therapist must be licensed in the <br />City; and <br />WHEREAS, one applicant has submitted an application and fees for a Therapeutic <br />Massage License renewal to continue to practice therapeutic massage in the City of <br />Mounds View; and <br />WHEREAS, the applicant is compliant under Chapter 514 of the Mounds View City <br />Code; and <br />WHEREAS, the applicant has submitted a Certificate of Liability Insurance in <br />accordance with the City Code; and <br />WHEREAS, the applicant's license is contingent on receiving a satisfactory <br />background check from the Minnesota Department of Criminal Apprehension. <br />NOW, THEREFORE BE IT RESOLVED, that Therapeutic Massage License renewal <br />is approved, contingent on a satisfactory background check, to the following below: <br />Name Location <br />Audrey Olesen Renting Space at Silver View Chiropractic - <br />2552 Count Rd 10 <br />NOW, MAY IT FURTHER BE RESOLVED, the renewal listed in the resolution will <br />be effective on January 1, 2010 and expire on December 31, 2010. <br />Adopted this 14t" day of December, 2009. <br />e Flaherty, <br />ATTEST: <br />~~ ~~ <br />James Ericson, Clerk-Administrator <br />(seal) <br />
