The applicant shali provide the City wiih a park dedication fee in the amount af <br />$100 per Code requiremenYs in Section 1204.02. <br />3. The applicax7t shalf provide the City with notarized lettees of permissioez from <br />William Culbert and Richard Edin, owners of 7532 an� 7540 Silver Lake Road, <br />respectively. . <br />4. The applicant shall record the �nal resotution, revised Certiii�ate of Survey, and <br />revised drainage and utility easement documents with Ramsey County within sixty <br />--� <u t) {30) days of final adoption, and present proof of such recording to the City of <br />Mounds View, or this approval shall be nutl and void. <br />5. The applicant shali include the foilowing restrictions in the deeds of the two <br />parceis and as part of the easements: <br />a. If one dweiling unit is burned or destroyed, it shall be reconstructed in a ;:<'<`� <br />uniform appearance. _: <br />h. If both dwelling units are burned or destroyEd, minimum lot widths shall <br />then prevaii as for single family homes. <br />c. A double dwelling unit may be rebuil� meeting the original conditions of <br />this code. <br />d.
