The a�plicant shall rovide at the time o�' application. su�'icieni <br />i►�'ormation to sho�v that construcYion insiallation and maintenance o� the antenna and <br />tower will not create a safet hazard or dama e to the ro e of other ersons. The <br />buildin� vermit at� lication must include a re ori and�lan from a qualified and re�istered <br />en�ineer or firm that s�ecifies and includes the followin�: <br />a. The tower height and desi n includin a cross section and elevation. <br />b. The hei ht above ade �or all otential mountin ositions for co-Ioeated antenna.e <br />and the minimum se aration distances beiween anter�ziae. <br />c. The ca�aacit of the towee, includin. the number and type of antennae #hat the tower <br />can accommoda#e. <br />d. The steps that the ap�Iicant will take to avoid interference with estabEished t�ublic_ <br />safet telecommunications. <br />e. An en�ineer's stam� and reQ-istration number. <br />f. An,y other information necessary for the Cit�to evaluate ihe re�uest._ <br />Subd. 3. Pt°oof of In�ur°a�c�e The a licant shall rovide the Cit with a co of the insurance <br />olic which rotects the residents and ro e owners from ersonal in°u or ro ert <br />dama e and holds harmless the Ci from an and all ciaims resultin from the <br />ct�nstruction oc coll�se of the tower antennae or accessorv eQUipment. <br />Subd. 4.
