<br />. <br /> <br />e <br /> <br />. <br /> <br />RESOLUTION NO. 4469 <br /> <br />CITY OF MOUNDS VIEW <br />COUNTY OF RAMSEY <br />STATE OF MINNESOTA <br /> <br />A RESOLUTION REESTABLISHING THE POLICY FOR PAYMENT OF HEALTH <br />CLUB MEMBERSHIPS IN EXCHANGE FOR SICK LEAVE <br /> <br />WHEREAS, in January of 1991, the City Council authorized that employees <br />who exercise on a regular basis and are physically fit will be healthier and more <br />productive~ <br /> <br />NOW THEREFORE BE IT RESOLVED that the City of Mounds View will allow <br />employees to continue to use their accumulated sick leave to pay for a health club <br />membership provided that the employee maintains a sick leave balance of eighty <br />hours at all times; <br /> <br />BE IT FURTHER RESOLVED that on a monthly basis the amount of sick leave <br />that may be used for the health club membership may not exceed a value of <br />$40.00; <br /> <br />BE IT FURTHER RESOLVED that the Finance Department will reimburse <br />employees for health club memberships upon receipt of a properly completed form <br />to be provided by the Finance Department and upon receipt of evidence of <br />payment by the employee to a health club; <br /> <br />BE IT FURTHER RESOLVED that memberships most be with a health club <br />that offers a diverse number of exercise activities (i.e., swimming, weight training, <br />aerobics, exercise machines, tennis racquetball, etc.); <br /> <br />BE IT FURTHER RESOLVED that golf, tennis, or racquetball club <br />memberships or lessons, martial arts or other special skilled activities are not <br />eligible activities as they do not provide the exercise program characteristics that <br />health clubs, such as U.S. Swim & Fitness Centers, the YMCA or YWCA, <br />Northwest Racquet & Swim Health Clubs, and other similar clubs; <br /> <br />Adopted this 27th day of December,. 1993 <br /> <br />(SEAL) <br /> <br /> <br />Attest: <br />
