<br />. <br /> <br />. <br /> <br />'. <br /> <br />RESOLUTION NO. 4045 <br /> <br />CITY OF MOUNDS VIEW <br />COUNTY OF RAMSEY <br />STATE OF MINNESOTA <br /> <br />A RESOLUTION ESTABLISHING A POLICY FOR PAYMENT OF <br />HEALTH CLUB MEMBERSHIPS IN EXCHANGE FOR SICK LEAVE <br /> <br />WHEREAS, the Council believes that employees who exercise on <br />a regular basis and are physically fit will be healthier and more <br />productive employees; <br /> <br />NOW THEREFORE BE IT RESOLVED that the city of Mounds View <br />will allow employees to use their accumulated sick leave to pay <br />for a health club membership provided that the employee maintains <br />a sick leave balance of eighty hours at all times; <br /> <br />BE IT FURTHER RESOLVED that on a monthly basis the amount of <br />sick leave that may be used for health club membership may not <br />exceed six hours or be valued at more than $75; <br /> <br />BE IT FURTHER RESOLVED that the Finance Department will <br />reimburse employees for health club membership on a monthly basis <br />upon receipt of a properly completed form to be provided by the <br />Finance Department and upon receipt of evidence of payment by the <br />employee to a health club; <br /> <br />BE IT FURTHER RESOLVED that membership must be with a health <br />club that offers a diverse number of exercise activities(i.e., <br />swimming, weight training, aerobics, exercise machines, tennis, <br />racquetball, to name a few); <br /> <br />BE IT FURTHER RESOLVED that golf, tennis, or racquetball <br />club memberships or lessons, martial arts or other special skill <br />activities are not eligible activities as they do not provide the <br />exercise program characteristics that health clubs, such as U.S. <br />Swim & Fitness Centers, the YMCA or YWCA, Northwest Racquet, Swim <br />& Health Clubs, and other similar clubs, provide. <br /> <br />Adopted this 22nd day of January, 1991. <br /> <br />(SEAL) <br /> <br /> <br />ATTEST: <br /> <br />RiC~ng Clerk-Administrator <br />
