RESOLUTION 8280 CITY OF MOUNDS VIEW COUNTY OF RAMSEY STATE OF MINNESOTA APPROVING A CONSTRUCTION CONTRACT WITH DAVE PERKINS CONTRACTING, INC. FOR 2014 BELLE LANE — PINEWOOD STORM SEWER REPAIR PROJECT WHEREAS, the City established a repair program whereby municipal utilities would be repaired on an as needed basis; and WHEREAS, A 140' section of existing 48" poly ethylene pipe sewer has been pushed to the surface requiring repair; and WHEREAS, Public Works prepared contract documents in house for the repair work in accordance with industry standards, and solicited and received 3 quotes as follows: Rank Contractor Quote Low Dave Perkins Contracting, Inc. $54,895 2 _ Penn Contracting $61,470 3 Northwest Asphalt $78,064 WHEREAS, Dave Perkins Contracting, Inc. of Ramsey, Minnesota submitted the lowest total combined quote in the amount of$54,895; and WHEREAS, City staff recommends approving a construction contract with Dave Perkins Contracting, Inc. to perform the rehabilitation work. NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of Mounds View, Ramsey County, Minnesota as follows: 1. The quote proposal for $54,895 submitted by Dave Perkins Contracting, Inc. of Ramsey, Minnesota for the 2014 Belle Lane — Pinewood Storm Sewer Repair Project (City Project No. 2014-M05) is hereby approved. 2. Public Works is directed to prepare a construction contract agreement for said project at said price. 3. The Mayor and City Administrator are hereby authorized and directed, for and on behalf of the City, to execute and enter into a construction contract agreement with Dave Perkins Contracting, Inc. 4. Monies for the Project shall be funded by the Storm Water Enterprise Fund (745- 4415-5150) in an amount not to exceed $63,130.00, which includes a 15% construction contingency of $8,235.00 for minor work associated with Project unknowns.
