<br />. <br /> <br />. <br /> <br />. <br /> <br />RESOLUTION No. 6397 <br /> <br />CITY OF MOUNDS VIEW <br />COUNTY OF RAMSEY <br />STATE OF MINNESOTA <br /> <br />RESOLUTION ESTABLISHING CITY CONTRIBUTION LEVELS FOR EMPLOYEES <br />ELECTING THE HEALTH SAVINGS ACCOUNT OPTION IN THE EMPLOYEE <br />BENEFITS PROGRAM <br /> <br />WHEREAS, the City Council has expressed its desire to make a high deductible <br />insurance/health savings account (HSA) combination available to the employees as an <br />alternative to the existing health maintenance organization (HMO) insurance plans; and <br /> <br />WHEREAS, the City's current HMO insurance options are provided by the State of <br />Minnesota Public Employee Insurance Program (PEIP); and <br /> <br />WHEREAS, PEIP will begin providing a HSA plan on January 1,2005 which can <br />be offered to the employees alongside the existing HMO insurance options; and <br /> <br />WHEREAS, it is necessary to establish the level of the City contribution to the HSA <br />option in order for employees to make informed choices between the HSA and HMO plans. <br /> <br />NOW, THEREFORE, BE IT RESOLVED, by the Mounds View City Council that <br />employees electing the HSA insurance option shall receive the same City contribution as <br />the employee would receive if they had elected the HealthPartners HMO option. <br /> <br />Adopted this 22nd day of November, 2004 <br /> <br />ATTEST: <br /> <br />~ <br />//~ b. t-.SJ <br /> <br />Kurt Ulrich, City Administrator <br /> <br />(seal) <br />
