<br />RESOLUTION NO. 6654 <br />CITY OF MOUNDS VIEW <br />COUNTY OR RAMSEY <br />STATE OF MINNESOTA <br />RESOLUTION APPROVING A NEW HEALTH INSURANCE PROVIDER FOR 2006 <br />WHEREAS, the City has purchased HeaithPartners Health Insurance for 2005; <br />WHEREAS, -the City utilized Joli'nson McCann to survey the market for 2006; and <br />~~ <br />WHEREAS, Johnson McCann received quotes from HealthPartners, Blue Cross/Blue <br />Shield and Service Cooperatives (Appletree Institute); and <br />WHEREAS, the apparent low rate is with Service Cooperative (Appletree Institute), who <br />uses Blue Cross/Blue Shield; and <br />WHEREAS, the Service Cooperative (Appletree Institute} has an annual premium of <br />$452,778/year; and <br />WHEREAS, the annual premium with the Service Cooperative (Appletree Institute} <br />would be an increase of 7.76% from the current HealthPartners 2005 rate; and <br />WHEREAS, Johnson McCann received compensation for their services as a <br />commission on the insurance policies paid from the health insurance company to Johnson <br />McCann. <br />NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of Mounds View <br />that a contract with a Service Cooperative {Appletree Institute) to provide employee group <br />health insurance is hereby approved and the Mayor and City Administrator are authorized to <br />sign the agreement. <br />Adopted this 24th day of October, 2005. <br />Ro arty, Mayor <br />• <br />ATTEST: /~ ~ G' ~T <br />~'~/` ~ <br />Kurt 111rich, City Administrator <br />(sea{) <br />
