On site, supportive childcare, as well as enrollment in head-start programs <br /> where appropriate. <br /> 2. On site supportive services <br /> 3. Community building activities <br /> 4. Flexible lease, allowing resident placement in in-patient treatment programs <br /> when necessary without jeopardizing the permanent housing lease. <br /> 5. Support systems for children including various support programs while parent <br /> is in treatment <br /> The Salvation Army believes that it is necessary to offer a broad range of support services <br /> in order to stabilize the lives of these difficult to serve families. The intent of this program <br /> is to both improve the present living situation of these families and, perhaps more <br /> important, to provide children with a solid foundation so that they can lead stable lives in <br /> the future. <br /> F. Non-Housing Support Services <br /> This building will be a supportive housing project. The concept is to provide enough on- <br /> site support services so that the residents, all of whom will move in with multiple problems <br /> and issues, will succeed in maintaining permanent housing and achieving.stability. <br /> Services offered will include on-site child care, transportation, case management, chemical <br /> dependency counseling, health care coordination, TB screening, resident-led recovery <br /> group meetings and social activities. All staff will be experienced in working with <br /> homeless, chemically dependent persons and persons of color. <br /> Participant's service and housing needs will be assessed and tracked by stag starting with <br /> the resident selection process and continuing through the entire time residents live in the <br /> Family Supportive housing campus. Residents referred to the project will be selected <br /> using an established intake process that includes both an application and interview. This <br /> process will ensure that applicants meet the program criteria(mcluding being homeless) <br /> and will solicit information regarding drug use,treatment histories, mental health as well <br /> as medical histories. This information will then be used to establish a case management <br /> plan as necessary. A case manager will be on-site full-time and will work with each family <br /> as necessary. <br /> Women With a Point- a Ryan White funded harm reduction organization will provide <br /> harm reduction based education, outreach and counseling support to program participants <br /> as well as education and training to staff. <br />
