SSSP – B, 2720 County Rd 10 – red gnome dogwood qty. 37 <br /> <br />Price is to include one application of hardwood mulch to all landscaped areas in spring, mulch is <br />provided at the Mounds View Public Works Yard. <br /> <br />Number of hours required for mowing ________ <br /> <br />Cost per hour for mowing $________ <br /> <br />Number of hours anticipated for perennial bed work _________ <br /> <br />Cost per hour for perennial bed work $_________ <br /> <br />City Wide Contract Specifications: <br />Due to the nature of working near a roadway, all personnel performing these services for the City of <br />Mounds View must wear a “Class 3, Level 2” or better safety vest with reflective striping, and meet any <br />and all other safety rules and regulations mandated by law. <br /> <br />When working in the roadway, safety precautions laid out by the Minnesota Manual on Uniform Traffic <br />Control Devices (MUTCD) must be followed. If traffic is impeded or obstructed, proper lane closures will <br />be necessary. Contractor will need to work with Ramsey County Public Works to obtain the proper <br />permits for lane closures. Mounds View city staff must be notified at least two full business days before <br />a traffic obstruction will be allowed. If work is being completed on the landscape areas without a traffic <br />impact (i.e. all contractor vehicles are parked legally out of the flow of traffic), fluorescent orange rollup <br />signs stating “road work ahead” or simply “workers” shall be placed ahead of the area where people are <br />working, on the right side of the road out of traffic, warning vehicles coming from each direction. <br /> <br />All harvested plant material may be unloaded at the Ramsey County Yard Waste Center on Long Lake <br />Road. Plant material from other non-contract sites may not be dropped off at the yard waste site except <br />as allowed during normal site operation hours. <br /> <br />All collected trash may be put in the dumpsters in the Mounds View Public Works yard at 2466 Bronson <br />Drive. Trash from other non-contract sites may not be dropped off here. No plant waste shall be put in <br />the dumpster. <br /> <br />A scouting report shall be filed weekly with city staff reporting the work that was completed and general <br />conditions of the landscape, please use the attached scouting form. Monthly payments for services will <br />be withheld until scouting reports are turned in and current. Future arrangements will be made to either <br />drop off the form in person, fax or email. <br /> <br />An insurance certificate naming the City of Mounds View and Ramsey County as ‘additional insured’ <br />shall be provided prior to approval of the contract. Required coverage and limits shall include: <br />• Workers’ Compensation – statutory <br />• General Liability - $2,000,000 <br />• Automobile Liability - $1,000,000 <br />• Excess or Umbrella Liability - $2,000,000
