As a review, the following is required for this type of license: 1. Completed Therapeutic Massage Enterprise Application 2. Completed Individual Therapeutic Massage Application 3. Provide evidence of your legal work status in the United States 4. Provide evidence of a diploma or certificate of graduation from a recognized school 5. Provide evidence of insurance coverage with liability limits of at least $1,000,000 for professional liability in the practice of massage 6. Provide documentation of 70 hours of resident study 7. In addition to the 70 hours of resident study, you must provide evidence of completing 400 hours of certified therapeutic massage training as part of earning a diploma or certificate from a recognized school 8. Provide evidence of having at least 1 year of experience practicing massage therapy as established by an affidavit 9. If your business is located at your home residence, and you are not the property owner, then the City requires written consent from the property owner allowing use of the property as a Therapeutic Massage business location. 10. Payment for the Therapeutic Massage Enterprise license, Individual Therapeutic Massage License and Investigation Fees Eric Rautio has the necessary insurance coverage in accordance with the City Code, received his training through CenterPoint Massage and Shiatsu Therapy School in St. Louis Park, and completed 70 hours of resident study, and has over 750 hours of certified therapeutic massage training, and provided evidence of his legal work status in the United States. In addition, Eric Rautio has been practicing massage therapy since 2013. (affidavit is on file with the City). All application materials and license fees (Therapeutic Massage Enterprise License and Individual Therapeutic Massage License) have been paid in full. Eric Rautio is the homeowner, and will be the only massage therapist at his home. His business name is Eric Roy Massage LLC.
