<br /> <br /> <br />RESOLUTION NO. 8485 <br /> <br />CITY OF MOUNDS VIEW <br />COUNTY OF RAMSEY <br />STATE OF MINNESOTA <br /> <br />RESOLUTION APPROVING SEVERANCE PAYMENT TO <br />PLANNING ASSOCIATE HEIDI HELLER <br /> <br /> <br /> WHEREAS, Heidi Heller submitted her resignation and her last day of employment <br />was October 30, 2015; and <br /> <br /> WHEREAS, the balance of accrued and unused sick leave time is 405.92 hours; <br />and <br /> <br />WHEREAS, the balance of accrued and unused vacation is 111.16 hours; and <br /> <br /> WHEREAS, Heidi Heller’s current rate of pay is $31.49 per hour; and <br /> <br />WHEREAS, in accordance with Section 3.45 of the Mounds View Personnel <br />Manual, the maximum potential payout of accrued/unused sick time will be 405.92 hours x <br />50% x her hourly rate of $31.49 = $6,391.21into Heidi Heller’s Retirement Health Savings <br />Plan (RHSP); and <br /> <br /> WHEREAS, in accordance with Section 3.47 of the Mounds View Personnel <br />Manual, the maximum potential cash payout of accrued/unused vacation time will be <br />111.16 hours x her hourly rate of $31.49= $3,500.43. <br /> <br /> NOW, THEREFORE, BE IT RESOLVED that the Mounds View City Council <br />approves a one-time payout of sick time not to exceed $6,391.21into Heidi Heller’s RHSP <br />in accordance with Section 3.45 of the Mounds View Personnel Manual. <br /> <br /> NOW, BE IT FURTHER RESOLVED that the Mounds View City Council <br />approves a one-time cash payout of vacation time not to exceed $3,500.43 in <br />accordance with Section 3.47 of the Mounds View Personnel Manual. <br /> <br />Adopted this 23rd day of November, 2015. <br /> <br /> <br /> ________________________________ <br /> Joe Flaherty, Mayor <br />ATTEST: <br /> <br /> ________________________________ <br /> James Ericson, City Administrator <br />(seal) <br /> <br />
