REQUEST FOR PROPOSAL UNDER THE <br /> MINNESOTA HOUSING FINANCE AGENCY • <br /> 1997 MINNESOTA CITY PARTICIPATION (MCPP) • <br /> Seven-County Twin Cities Metropolitan Area <br /> (Anoka, Carver, Dakota, Hennepin, Ramsey, Scott or Washington counties) <br /> The following is a Request for Proposal (RFP) under MHFA's Minnesota City Participation <br /> Program. This RFP is in outline format, specifying questions that must be answered. <br /> Applications must be submitted in this format and all questions must be answered. Final <br /> determination of the total amount of program funds available and individual allotments will be <br /> made by MHFA in compliance with a per capita distribution method specified in statute. <br /> Questions may be directed to Michelle Ramos, Minnesota Homes Division at 612-297-5146 <br /> (Metro) or 1-800-710-8871 (Greater Minnesota). <br /> Applications must be received by MHFA by 4:00 p.m., February 18, 1997 for <br /> funding consideration under this program phase. <br /> I. ELIGIBLE ORGANIZATION <br /> A. Name and address of your organization. <br /> B. Name and phone number of your contact person. <br /> C. Type of organization (CityDepartment, Port Authority, HRA, orEDA). <br /> II. ORIGINATING LENDER <br /> Cities participating in the MCPP MUST PROVIDE A LETTER from at least <br /> one lender from the enclosed Minnesota Mortgage Program lender list who <br /> will take mortgage applications and process loans for your program. <br /> Cities have two options when selecting lenders to originate loans: <br /> 1) Option A: Cities may allow all lenders from the enclosed list to originate the city's <br /> MCPP loans <br /> OR <br /> 2) Option B: Cities may choose to restrict participation to only select lenders of their <br /> choice from the lender list. <br /> If selecting Option B, please list: 1) specific lenders chosen; 2) their <br /> addresses; 3) their phone numbers; 4) their fax numbers; and 5) their <br /> contact persons. PLEASE NOTE: BE PRECISE WHEN LISTING LENDER <br /> INFORMATION, MHFA WILL PRINT BROCHURES USING THE EXACT <br /> INFORMATION YOU PROVIDE TO US. <br /> Please indicate SPECIFICALLY which option your city has chosen. • <br /> Additional lenders may NOT be added later in the program term. <br />
