SMALL BUSINESS WEEK AWARDS April 30 -- May 6, 199b SMALL BUSINESS PERSON OF THE YEAR w- 1995 INTRODUCTION Each year our nation recognizes its outstanding small business • owners for their personal achievements and contributions to their communities. These entrepreneurs are recognized in each state as the Small Business Person of the Year. They are then eligible for consideration for the national Small Business Person of the Year Award and willbe invited 1-o attend Small Business Week' , activities in Washington, B.C Orri nrc` the' weer of April "?0 ' 14,7-1v- 6 , 1995.. All nominees for the Small Business Person of the Year Award muf be residents of and employed in the United States and will be subject to background checks. All applicants must comply with federal civil rights laws. Individuals who own and operate or 410 bear principal responsibility for operating a small business are eligible. Partners who jointly own and operate a small business may, be nominated as a "team" so long as the number of individuals in a single nomination does not ` exceed.,three. The U.S. Small, Business Administration size standards apply in defining a business as small (refer to size standards included in this awards package) . NOMINATIONS The nomination package must include (in the following order) : - A single cover page with: * Nominee's full name, title, business and home addresses 1 and telephone numbers. * Award Category (Small Business: Person of the Year) . * The nominator's name, title, name of business/organization and business address and telephone number. * A one-paragraph description of the nominee's business. - A completed SBA Form 898 (biographical data form) , package For !f{--Cam!! -nominations Cllt/1UjCG 1.1 ill 1..13Av LJ A + ldy +• � +•••- -- -•-� / -- form 898 must be submitted for each person. - An original black and white photo of the nominee, either 8" x 10" or 5" x 7" (xerographic copies are not acceptable) .
