As our information packet describes, we must receive from you a resolution from your <br /> governing board(model attached), a completed questionnaire (enclosed), and a timetable for <br /> completion of your plan. <br /> The information we require from you is critical to our effort to plan regional systems in your <br /> 410 <br /> area.
