<br />RESOLUTION NO. 6678 <br /> <br />CITY OF MOUNDS VIEW <br />COUNTY OF RAMSEY <br />STATE OF MINNESOTA <br /> <br />RESOLUTION APPROVING NEW DENTAL AND HEALTH INSURANCE <br />PROVIDERS FOR 2006 <br /> <br /> <br /> WHEREAS, the City has purchased HealthPartners Dental and UNUM <br />Life Insurance in 2005; and <br /> <br /> WHEREAS, the City utilized Johnson McCann to survey the market for <br />2006; and <br /> <br /> WHEREAS, the apparent low rate for dental insurance is Delta Dental; <br />and <br /> <br /> WHEREAS, the apparent low rate for life insurance is Ochs, Inc. <br /> <br /> NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of <br />Mounds View that a contract with Delta Dental for dental insurance and Ochs, <br />Inc., for Life Insurance is hereby approved and the Mayor and City Administrator <br />are authorized to sign the agreement. <br /> <br /> Adopted this 14th day of November, 2005. <br /> <br /> <br /> <br /> __________________________________ <br /> Rob Marty, Mayor <br /> <br />ATTEST: <br /> <br /> __________________________________ <br /> Kurt Ulrich, City Administrator <br /> <br />(seal)
