RESOLUTION NO. 7161 <br />CITY OF MOUNDS VIEW <br />COUNTY OR RAMSEY <br />STATE OF MINNESOTA <br />APPROVING A THERAPEUTIC MASSAGE LICENSE <br />TO XIUJU LI, DOING BUISNESS AS JIAN KANG MASSAGE, LLC. LOCATED AT 2552 <br />COUNTY ROAD 10 IN MOUNDS VIEW <br />WHEREAS, the City Code requires that massage therapist must be licensed in the City; <br />and <br />WHEREAS, the City Code requires that the applicant have one year of experience <br />practicing massage therapy as established by an affidavit and can document within two years of <br />obtaining the license that the applicant has completed four hundred hours of certified <br />therapeutic massage training from a recognized school; and <br />WHEREAS, Xiuju Li is applying for a Therapeutic Massage License to practice <br />therapeutic massage at 2552 County Road 10 Mounds View; and <br />WHEREAS, Ms. Li received her training through America's Institute of Traditional <br />Medicine located in Rosemead, California, and completed 500 hours of training; and <br />WHEREAS, Ms. Li submitted an affidavit which stated that she has provided therapy to <br />family friends and the public for one year; and <br />WHEREAS, it is required that the applicant must have a satisfactory background check; <br />and <br />WHEREAS, Ms. Li has not submitted a Certificate of Liability Insurance for the <br />necessary insurance coverage in accordance with the City Code; and <br />WHEREAS, this Therapeutic Massage License will not be awarded unless staff <br />receives a satisfactory background check from the Minnesota Bureau of Criminal Apprehension. <br />NOW, THEREFORE BE IT RESOLVED, a Therapeutic Massage License is approved to <br />Xiuju Li, pending receipt of a Certificate of Liability Insurance and verification of affidavit. <br />NOW, MAY IT FURTHER BE RESOLVED, that Ms. Li's Therapeutic Massage license <br />will be effective on October 19, 2007 and expire on December 31, 2007. <br />Adopted this th day of September, 2007. <br />Rob Marty, Mayor <br />ATTEST: ~~ ~~ <br />Jim Ericson, Interim City Administrator <br />(seal) <br />
