10/16/2020 Revize Online Forms
https://moundsviewmn.rja.revize.com/reports/76910 2/2
PRIVACY NOTICE: The information provided by you on this application will be used to determine your suitability for appointment to an advisory
commission. Participation as an advisory commission member is strictly voluntary and you are not required by law to provide this information. However, should you
not furnish this information the City may have difficulty determining your suitability for appointment, contacting you regarding your information, and if selected,
notifying you of your duties on the advisory commission. Under Minnesota State Statute 13.601, subd.3, the following information is considered public information
before appointment: name, city of residence, education and training, employment history, volunteer work, awards and honors, prior government service, veteran
status. Once an individual is appointed, the following additional items of data are considered public: residential address, telephone number, email address. I have
read and understand the Tennessean Warning and certify that the statements in this application are true and correct to the best of my knowledge:
The City of Mounds View is committed to the policy that all persons shall have access to its programs, facilities and employment without regard
for race, ethnicity, gender, age or physical abilities.
father and I have run this business for over 22 years. Our office is currently located in Mounds View.
Memberships, Accomplishments or Other Qualifications
I have been a PTA president and volunteer coordinator for a community center as well as the MS Society. I enjoy volunteering and help
wherever I can. I am currently working with some volunteer coordinator tasks for Rein in Sarcoma. I also worked very heavily with the levy
groups when my kids were in school in the district. I am a graduate of Irondale High School and have enjoyed being a part of the school
district when my kids attended the same schools that I did and even had some of the same teachers I did.
What is your reason for wanting to service on this Commission/Committee
There are so many reasons but the main one is to be better informed about what is going on in the city. It’s easy to set back and give
commentary about things that the city is doing, but if you aren’t properly informed then you don’t have all the facts. I also want to use my
business skills in other ways and hopefully I can with this committee. I would very much look forward to the opportunity, the challenges,
contribute more to the Mounds View Community and be able to share that with others.
Please attach additional information if desired:
No file chosenChoose File
Optional
Applicant Signature
Lisa Marquis
Date
10/15/2020
Format: MM/DD/YYYY
