of Mounds View Memo <br />To: Honorable Mayor and City Councill <br />From: Kurt Ulrich, City Administrator <br />Date: 1/24/05 <br />Item Title/Subject: DRAFT Community Survey <br />Attached are to drafts of the community survey that is scheduled for review at the <br />February 7th Council work session. The first survey is the prescribed length of around 50 <br />questions, while the second survey is 104 questions. The longer survey is a first draft of <br />possible questions after review by the department heads. Obviously, the questionnaire <br />had to be reduced significantly to get to the 50 -question level. <br />I thought the longer version may be of interest if you. Perhaps you'll see a question that <br />you would like to substitute into the short version. It is the intent to keep the survey at the <br />budgeted amount of 50 questions. Additional questions are $135 each. Please review <br />the documents for discussion on the 7th. Thank you. <br />
