G. Source of local funding share (identify specific sources and amounts of public and/or private <br /> funds on hand to undertake the soccer project). <br /> H. Proof of property ownership (supply documentation of fee title, easement or long-term lease <br /> applicable to the proposed site). <br /> I. Operations and maintenance plan (identify who will provide these services and the long-term <br /> source of funding). <br /> J. Describe, in general, how the facility will be programmed and scheduled (identify access <br /> priorities). <br /> K. Identify and summarize any cooperative agreements with other governmental jurisdictions <br /> relating to the proposed soccer facility. <br /> L. Describe the timeline for project implementation. <br /> M. Identify when (month and year) the project will be open for public use. (Note: Soccer field <br /> development under this program must be available for public use within two years of a <br /> development partnership grant award.) <br /> N. Include a resolution from the governing body of the jurisdiction making application <br /> authorizing the submittal of the application and accepting the terms as outlined in the <br /> request for grant applications. <br />
