If a Permitee contracts the services of a licensed beverage/restaurant services, they <br /> would need to supply the above requirements. <br /> KITCHEN USE <br /> Kitchen facilities include: refrigerator, freezer, ice maker, convection ovens (2), gas cooktop, gas <br /> oven, etc. <br /> It is the policy of the Mounds View Community Center that only commerci8ally prepared foods <br /> be served at events. No "Pot Luck" type events are allowed. <br /> A Food Catering information form is required for all events serving food. This form should be <br /> returned to the Community Center 4 weeks prior to your event. <br /> Commercially prepared foods must be transported in approved containers. Kitchen is then used <br /> as a staging/warming area. <br /> Groups providing food for public events must secure a concession permit with Ramsey County <br /> Health Department. Thoses permits must be applied for at least 6 weeks prior to the event. <br /> Only serving utensils and dishes are available for use. Plates, glasses, cups, knives, forks and <br /> spoons will need to be supplied. <br /> BUILDING CAPACITY <br /> • To conform with Fire Code Specifications, the Mounds View Community Center must limit the <br /> capacity of people in each room. Seating limits are: <br /> Main Hall - <br /> SUPERVISION <br /> A building facility manager must be present during the event. The Facility Manager fee is $10.00 <br /> and must be paid as part of the Rental Fee. Parks and Recreation Department will arrange for <br /> Facility Manager. <br /> LIABILITY <br /> The Permitee using the Community Center shall agree to compensate the City of Mounds View <br /> for all damages to facilities, equipment or other property owned by the City to compensate an <br /> employee for damage to personal property by any person(s) attending the event and further <br /> assumes all liability for any personal injuries, including death caused by participants on the <br /> scheduled event. <br /> If beer/wine are served, the Permitee, or contracted service vendor paid by the Permitee, must <br /> • supply the City with a certificate of insurance, naming the City as an additional insured, showing <br /> effective coverage of public liability insurance in the amount of$100,000 for a single claim and <br />
