RESOLUTION NO.9892 <br />CITY OF MOUNDS VIEW <br />COUNTY OF RAMSEY <br />STATE OF MINNESOTA <br />RESOLUTION APPROVING A THERAPEUTIC MASSAGE <br />THERAPIST LICENSE FOR CRYSTAL HOON <br />WHEREAS, the City Code requires that a massage therapist must be licensed to <br />perform massage therapy in the City of Mounds View; and <br />WHEREAS, Crystal Hoon is applying for a Therapeutic Massage Therapist License <br />to conduct continuing education classes at the Mounds View Community Center; and <br />WHEREAS, the City Code requires that the applicant have one year of experience <br />practicing massage therapy as established by an affidavit and can document that the <br />applicant has completed seventy (70) hours of resident study and an additional four <br />hundred (400) hours of certified therapeutic massage training from a recognized school; <br />and <br />WHEREAS, Ms. Hoon has completed the necessary application materials, provided <br />proof of massage training hours and proof of legal status to work in the United States; and <br />WHEREAS, Crystal Hoon has a satisfactory criminal background check report by the <br />Mounds View Police Department; and <br />WHEREAS, Ms. Hoon shall provide evidence of liability insurance coverage in <br />accordance with Mounds View City Code. <br />NOW, THEREFORE BE IT RESOLVED that an individual therapeutic massage <br />therapist license be issued to Crystal Hoon. <br />BE IT FURTHER RESOLVED that the license may be revoked or denied by the City <br />Council based on negligence, nuisance, or violation Chapter 114 of the Mounds View City <br />Code. <br />BE IT FINALLY RESOLVED that the license will expire December 31, 2024, and <br />shall be renewed annually. <br />Adopted this 8th day of April, 2024. <br />ach Lindstrom, Mayor <br />ATTEST: Au <br />I-), / <br />Nyl ik nd, Ci y Administrator <br />(Seal) <br />The Morjnii5 V!ew Vision <br />A Tbrivin,- Desirable Commmniiy <br />
