E. PROFESSIONAL QUALIFICATIONS <br />1. The Contractor shall at all times maintain a force of qualified personnel to sufficiently <br />and effectively execute the contract. Personnel must possess the required Pesticide <br />License from the Minnesota Department of Agriculture. <br />2. The Contractor shall be a currently licensed tree contractor with the City of <br />Mounds View. <br />3. The Contractor shall have one employee that is an ISA Certified Arborist who will be <br />designated as the project manager. This employee shall manage crew activity, update <br />and maintain all paperwork related to the Services. <br />F. METHOD OF MEASUREMENT <br />1. The Contractor will provide a written summary table listing completed tree injection <br />work. Each entry shall list the property address, location description, date of <br />application, DBH, approximate volume of pesticide applied and any relevant comments <br />concerning the treatment. <br />2. Payment will be provided at the contract unit prices based on DBH (Diameter Breast <br />Height in inches). <br />3. Due to budget restrictions, the City reserves the right to determine the quantities of <br />trees to be treated. No adjustment on contract price per inch or additional compensation <br />will be made for alteration in quantities of the contract. <br />G. BASIS OF PAYMENT <br />1. Payment at the contract unit price for Public ash trees shall be full compensation for all <br />labor, materials, equipment, traffic control and other activities necessary to furnish and <br />apply EAB inhibiting pesticide as specified herein. <br />A-3 <br />MU125\11\729839.v2
