City of Mounds View <br />2401 Mounds View Blvd <br />Mounds View. MN 55112 <br />STIPULATIONS <br />The permit will be issued with the following stipulations as conditions of the permit. By accepting the permit, the Permittee <br />agrees to these stipulations: <br />GENERAL <br />1. An as -built survey of all stormwater BMPs (ponds, rain gardens, trenches, swales, etc.) is to be submitted to the City <br />for verification of compliance with the approved plans. <br />EROSION AND SEDIMENT CONTROL STIPULATIONS <br />2. Erosion control measures shall be in place prior to grading activities and maintained through project completion. These <br />features can include sediment logs, erosion blankets, sod, riprap, silt fence and temporary or permanent vegetation. Hay bales <br />are no longer recommended due to the impedance of water flow. <br />3. All exposed soils, including stockpiles, shall be stabilized within 14 days if not being actively worked and after the <br />completion of grading. Slopes steeper than 3:1 shall be stabilized within 7 days. <br />4. The City Inspector may require additional erosion control features, dependent on site condition. <br />5. Refer to the MPCA "Protecting Water Quality in Urban Areas" manual at http://www.pca.state.mn.us/gzgha87 for BMP's. <br />6. Please contact , the , at-_ -- if you have questions or to discuss site stabilization <br />practices. <br />GENERAL PROVISIONS <br />1. The project shall be in accordance with the plans most recently submitted and approved by the City as part of the record of <br />this project. <br />2. This permit is not assignable by the Permittee, except with the written consent of the City. <br />3. The Permittee shall grant access to the site at all reasonable times during and after construction to authorized <br />representatives of the City for inspection of the work authorized hereunder. <br />4. In all cases where the Permittee, by performing the work authorized by this permit, shall involve the taking, using, or <br />damaging of any property rights or interests of any other person or persons, or of any publicly owned lands or improvements <br />thereon or interests therein, the Permittee, before proceeding, shall obtain the written consent of all persons, agencies, or <br />authorities concerned, and shall acquire all property, rights and interests needed for the work. <br />5. This permit is permissive only. No liability shall be imposed on the City of any of its officers, agents, or employees, officially <br />or personally, on account of the granting hereof or on account of any damage to any person or property resulting from any act <br />or omission of the Permittee or any of its agents, employees, or contractors. This permit shall not be construed as estopping or <br />limiting any legal claims or right of action of any person against the Permittee, its agents, employees, or contractors, for any <br />damage or injury resulting from any such act or omission, or as estopping or limiting any legal claim or right of action of the City <br />against the Permittee, its agents, employees, or contractors for violation of or failure to comply with the permit or applicable <br />provisions of law. <br />6. Any stormwater management facilities approved as part of this permit shall be properly maintained in perpetuity to assure <br />that they continue to function as originally designed. <br />7. After vegetation is in place and erosion control features have been removed, notify the City Inspector. Once the Inspector <br />verifies that site conditions comply with all permit requirements, your cash surety will be returned to the remitter. <br />8. Failure to comply with the provisions of this permit is a violation of the law and may result in forfeiture of permittee's <br />surety. <br />Page 2 of 2 <br />
