RESOLUTION NO. 1665 <br />CITY OF MOUNDS VIEW <br />COUNTY OF RAMSEY <br />STATE OF MINNESOTA <br />RESOLUTION APPROVING WAIVER. OF GROUP HEALTH <br />INSURANCE COVERAGE AND ALLOWING RE-ENTRY <br />WHEREAS, Clerk -Administrator Pauley has advised the City <br />Council that his wife's employer provides a more attractive health <br />insurance program on a coverage and cost basis; and <br />WHEREAS, Clerk -Administrator Pauley has indicated a desire <br />to transfer his family coverage from the City of Mounds View group <br />to his wife's employer gro..r, <br />and <br />WHEREAS, Group Health Plan has agreed to allow such transfer <br />to occur without any underwriting requirements and at no expense to <br />the Mounds View group if the City of Mounds View agrees to allow <br />Mr. Pauley to transfer his family coverage back to the City of <br />Mounds View should his wife's employment conditions change; and <br />WHEREAS, it is the desire of the Mounds View City Council <br />to allow employees to improve their health insurance coverage at <br />no greater expense or risk to the City of Mounds View without <br />jeopardizing their ability to obtain coverage through the City of <br />Mounds View's Group Plan. <br />NOW, THEREFORE, BE IT RESOLVED by the City Council of the <br />City Mounds View that Clerk -Administrator Pauley be allowed to <br />re-enter his family health insurance coverage to the City of Mounds <br />View group at such time that the conditions of his wife's employ- <br />ment change in such a manner to make such a re-entry more attractive <br />or necessary. <br />ATTEST: <br />(SEAL) <br />Adopted this loth day of October, 1983. <br />Mayor <br />Clerk -Administrator <br />
