CITY OF MOUNDS VIEW, MINNESOTA <br />� <br />MAY 1, 1984-1985 I <br />Quotations are being received by the City of Mounds View until 12:00 P.M. on <br />April 23, 1984, at which Lime they will he opened and read aloud at the office <br />of the City's Finance Director in Mounds View, M4nnesota. An award will be <br />made at the City Council meeting on April 30, 1984. Quotations are being <br />requested on the following insurance: <br />1. Property <br />2. Valuable Papers R Records <br />3. Extra Expense <br />4. Inland Marine <br />5. Boiler R Machinery <br />6. Comprehensive General Liability <br />7. Comprehensive Automobile <br />(a) Liability <br />(b) Comprehensive <br />(c) Collision <br />8. Crime <br />9. Depositors Forgery <br />10. Bonds <br />11. Umbrella Liability <br />12. Workers' Compensation <br />13. Public Officials' Liability <br />All quotations shall be submitted on forms provided, together with the specifi- <br />cations, and specimen copies of the policies and endorsements must be included <br />With all proposals. They must be submitted to the City's Finance Director, <br />Mr. Donald Brager, by the date and time specified above. Any questions should <br />be directed to the office of Corporate Risk Managers, Inc., attention Mr. Brian <br />Colway, the City's Insurance Manager, telephone: (612) 937-8942. <br />The City of Mounds View reserves the right to waive any informalities in the <br />quotations; to reject any or all quotations or parts of quotations and to make <br />any award which it considers to be in the best interest of the City. <br />Agents submitting cost proposals must complete the certification statement on <br />the cost proposal form. Any variations to coverage provisions or policy <br />conditions must be identified by addendum. <br />e: <br />
