An organization desiring to obtain A state license to conduct lawful gambling must make <br />annual application to the board on forms provided by the board. <br />Saba.2. Coateai of poration. The application must contain the following information: <br />A. the official, legal same of the organization; <br />B. the business address; <br />C. the business telephone number; <br />D. the city or town, and county in which gambling site is located; <br />E. the address of thepmbling site, indicating whether the tire is or is not within city or town limits; <br />F. the name and title of organization's executive officers; <br />G. an indication of Minnesota or Internal Revenue Service tax exempt status; <br />H. an indication of the type of eligible organization; <br />1. an indication of whether the applicant is chartered by parent organization; <br />L an indication of length of existence in Minnesota; <br />K. the name, address, and date of birth of the gambling manager; <br />L. A verification of the required fidelity bond for gambling managers; <br />M. an indication of whether the organization owns or leases the gambling site; <br />N. an indication of the class of license applied for; <br />0. the numbnt of active members; <br />P. if incorporated, the number of the reticles of incorporation and where filed; . <br />Q. the federal I.D. number, if any: of the <br />R. an "authorization to inspect bank records' of the games of chance bank acco`ertupon the site Lwt observe the lawful <br />S. a consent that local law enforcement officers or agents of the board my enter <br />and <br />gambling being conducted and 10 enforce the law for any unauthorized game or p r <br />T. such additional information as is necessary to property identify the applicant and to ensure compliance with <br />Minnesota Statutes, sections 349.11 to 349.22. <br />Solap.3.
