ORDINANCE NO. 391 <br />PAGE, TWO <br />subdivision 3. Application The application shall <br />contain the Eollowing information together with any <br />additional information requested by the Citv: <br />(1) Full name and address of the <br />organization. <br />(2) A brief statement of the or anization's <br />purposes and activities. <br />(3) The length of time the organization <br />has been in existence. <br />(4) The number of active members. <br />(5) A brief description of the event or <br />activity for which the permit is bein <br />requested. <br />(6) The park proposed to be used for the <br />activity includin any facilities in the <br />park which will be used. <br />(7) The dates and times for which the permit <br />is requested. <br />(8) The anticipated number of participants <br />(9) The person or persons from the <br />organization who will be in responsible <br />charge of the event or activity and if <br />the sale of beer. <br />application shall be made to the Citv Parks and <br />s <br />nt <br />Subdivision 4. Permit Fee. The fee shall be <br />$50.00 and shall be paid in full at the time of <br />application. <br />
