CHERYL SUTTON IN ACCORDANCE WITH MOUNDS VIEW CITY CODE WHEREAS, on August 29, 1994, Cheryl Sutton was given a written request by her Department Head, Mike Ulrich, to provide written documentation from her physician for recent absences of more than three days; and WHEREAS, the written notice had a deadline of 3:30, September 1, 1994, to provide such documentation; and WHEREAS, the written request for the documentation followed a verbal request which had not received compliance; and WHEREAS, the request for such documentation is in accordance with Chapter 305, Subdivision 3 of the City Code; and WHEREAS, said documentation was not provided within the specified timeframe; and WHEREAS, on September 1, 1994, after consultation with the Mayor as per Chapter 307.03, Subdivision 2, Ms. Sutton was suspended for three working days without pay; and WHEREAS, the City Code, Chapter 307.03 , Subdivision 2 requires that the Council, within 7 days of a suspension, affirm by resolution said suspension. NOW, THEREFORE, BE IT RESOLVED THAT the City Council in and for the city of Mounds View does hereby affirm the three work day suspension without pay for Ms. Cheryl sutton which commenced at 9:00 a.m. on Friday, September 2, 1994 and ends at 9:00 a.m. on Thursday, September 9, 1994.
