STATE OF MINNESOTA
RESOLUTION APPROVING SEVERANCE FOR
JEREMIAH ANDERSON, HOUSING/CODE ENFORCEMENT INSPECTOR
WHEREAS, Jeremiah Anderson submitted his resignation and his last day of
employment was July 5, 2013; and
WHEREAS, the balance of accrued and unused sick leave time is 202.98 hours;
and
WHEREAS, the balance of accrued and unused vacation is 37.31; and
WHEREAS, Jeremiah Anderson's current rate of pay is $27.08 per hour; and
WHEREAS, in accordance with Section 3.45 of the Mounds View Personnel
Manual, the maximum potential payout of accrued/unused sick time will be 202.98 hours x
50% x his hourly rate of $27.08 = $2,748.35 into Mr. Anderson's Retirement Health
Savings Plan; and
WHEREAS, in accordance with Section 3.45 of the Mounds View Personnel
Manual, the maximum potential payout of accrued/unused vacation time will be 37.31
hours x 50% x his hourly rate of$27.08 = $505.18 into Mr.Anderson's Retirement Health
Savings Plan; and
WHEREAS, in accordance with Section 3.47 of the Mounds View Personnel
Manual, the maximum potential payout of accrued/unused vacation time will be 37.31
accrued/unused vacation hours x his hourly rate of$27.08 = $505.18; and
WHEREAS, in accordance with Section 3.47 of the Mounds View Personnel
Manual, the maximum potential payout of unused compensatory time will be .12
unused compensatory hours x his hourly rate of$27.08 = $3.25.
NOW, THEREFORE, BE IT RESOLVED that the Mounds View City Council
approves a one-time payout of sick time not to exceed $2,748.35 into Mr. Anderson's
Retirement Health Savings Plan in accordance with Section 3.45 of the Mounds View
Personnel Manual.
NOW, THEREFORE, BE IT FURTHER RESOLVED that the Mounds View City
Council approves a one-time payout of accrued/unused vacation time not to exceed
$505.18 into Mr. Anderson's Retirement Health Savings Plan in accordance with Section
3.45 of the Mounds View Personnel Manual.
NOW, THEREFORE BE IT EVEN FURTHER RESOLVED, that the Mounds View
City Council approves a one-time payout of vacation time not to exceed $505.18,
consistent with Section 3.47 of the Mounds View Personnel Manual.

NOW THEREFORE BE IT FINALLY RESOLVED, that the Mounds View City
Council approves a one-time payout of compensatory time not to exceed $3.25, consistent
with Section 3.47 of the Mounds View Personnel Manual.
