STATE OF MINNESOTA
RESOLUTION APPROVING SEVERANCE PAYMENT TO
STEVE DAZENSKI, PARKS SUPERVISOR
WHEREAS, Steve Dazenski submitted his resignation for retirement and his last day of
employment was December 4, 2013; and
WHEREAS, the balance of accrued and unused sick leave time is 610.36 hours; and
WHEREAS, the balance of accrued and unused vacation is 124.16 hours; and
WHEREAS, Steve Dazenski's current rate of pay is $31.28 per hour; and
WHEREAS, in accordance with Section 3.45 of the Mounds View Personnel Manual, Mr.
Dazenski is qualified to contribute 1,280 sick hours to his RHSP; and
WHEREAS, from 2008 to present, Mr. Dazenski has contributed 794.25 sick hours to his
RHSP; and
WHEREAS, due to the base cap on sick hours contributed to the RHSP, Mr. Dazenski is
allowed to contribute a maximum of 485.75 sick hours into his plan; and
WHEREAS, in accordance with Section 3.45 of the Mounds View Personnel Manual, the
maximum potential payout of accrued/unused sick time will be 485.75 hours x 65%x his hourly rate
of$31.28 = $9,876.27 into Mr. Dazenski's Retirement Health Savings Plan; and
WHEREAS, in accordance with Section 3.47 of the Mounds View Personnel Manual, the
maximum cash potential payout of accrued/unused vacation time will be 124.16 hours x his hourly
rate of$31.28 = $3,883.72.
NOW, THEREFORE, BE IT RESOLVED that the Mounds View City Council approves a
one-time payout of sick time not to exceed $9,876.27 into Steve Dazenski's Retirement Health
Savings Plan in accordance with Section 3.45 of the Mounds View Personnel Manual.
NOW, BE IT FURTHER RESOLVED that the Mounds View City Council approves a one-
time cash payout of vacation time not to exceed $3,883.72 to Steve Dazenski in accordance with
Section 3.47 of the Mounds View Personnel Manual.
