Minnesota <br /> Association for <br /> Volunteer <br /> Administration <br /> 9olldrtqurzvmks,o, <br /> MEMORANDUM OF UNDERSTANDING <br /> Between the Minnesota Association for Volunteer Administration (MAVA) <br /> and <br /> Mounds View <br /> MAVA is looking forward to working with Mounds View as part of the MAVA Small Grants for <br /> Starting or Enhancing a City Volunteer Program. Please have the appropriate City leadership <br /> sign and date two copies of the following and mail to: MAVA, 1800 White Bear Ave. N., <br /> Maplewood, MN 55109 or scan and e-mail to office@mavanetwork.org. We will sign and return <br /> one copy to you. After receipt of signed agreement, MAVA will process grant award. <br /> Grant period: September 1, 2014 —August 30, 2015 <br /> 1. MAVA will provide: <br /> • $2,000 grant. <br /> • Complimentary attendance for up to two staff to attend the MAVA The Volunteer Impact <br /> Leadership Training Series and other full or part day workshops MAVA offers during the <br /> grant period at no cost. <br /> • Up to four hours of MAVA consulting time for your volunteer program. <br /> • A complimentary MAVA membership for the period of the grant, renewable at the end of <br /> the grant period <br /> 2. As a condition of a total subgrant of$2,000 from the Minnesota Association for Volunteer <br /> Administration of a grant from the Bush Foundation, Mounds View (City) agrees to: <br /> • Submit a brief work plan for City volunteer engagement by November 1, 2014. (An <br /> update on the work plan submitted in the proposal). <br /> • Participate in 3-4 phone meetings with other pilot sites to share progress and learn from <br /> each other. <br /> • Submit a brief report on the initial lessons learned by March 31, 2015. <br /> • Submit at least three documents used in the volunteer program with permission for MAVA <br /> to share those with other cities by August 1, 2015. <br /> • Submit a brief final report by September 30, 2015. MAVA will provide a report format.The <br /> report would include progress on the work plan, number of volunteers involved and <br /> departments utilizing volunteer during the grant period, the results of short two/three <br /> question survey given to 3-5 volunteers and/or City staff on results of the effort, lessons <br /> learned and feedback on on what resources were the helpful to your city. <br /> • Do a brief pre and post assessment of volunteer engagement. <br /> • Respond to a follow up phone call two years after grant period on volunteer program <br /> progress. <br /> 3. MAVA reserves the right, in its sole discretion, to discontinue funding if: (a) it is not <br /> satisfied with the progress of the activity to be supported by the grant; (b) City has failed in <br /> any other material way to comply with the terms of this agreement; (c)we determine that <br /> any statement in your proposal that we relied on in approving the grant is false. City <br /> agrees to repay to MAVA any grant funds that have not been used by the end of the grant <br /> period in compliance with the terms of this agreement. <br /> 4.
