CITY OF MOUNDS VIEW
COUNTY OR RAMSEY
STATE OF MINNESOTA
RESOLUTION APPROVING A NEW HEALTH INSURANCE PROVIDER FOR 2006
WHEREAS, the City has purchased HeaithPartners Health Insurance for 2005;
WHEREAS, -the City utilized Joli'nson McCann to survey the market for 2006; and

WHEREAS, Johnson McCann received quotes from HealthPartners, Blue Cross/Blue
Shield and Service Cooperatives (Appletree Institute); and
WHEREAS, the apparent low rate is with Service Cooperative (Appletree Institute), who
uses Blue Cross/Blue Shield; and
WHEREAS, the Service Cooperative (Appletree Institute} has an annual premium of
$452,778/year; and
WHEREAS, the annual premium with the Service Cooperative (Appletree Institute}
would be an increase of 7.76% from the current HealthPartners 2005 rate; and
WHEREAS, Johnson McCann received compensation for their services as a
commission on the insurance policies paid from the health insurance company to Johnson
McCann.
NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of Mounds View
that a contract with a Service Cooperative {Appletree Institute) to provide employee group
health insurance is hereby approved and the Mayor and City Administrator are authorized to
sign the agreement.
