CITY OF MOUNDS VIEW
COUNTY OF RAMSEY
STATE OF MINNESOTA
APPROVING CHANGE ORDERS N0.4-6 AND AUTHORIZING INITIATION OF
HVAC CHANGES FOR THE CITY HALL REHABILITATION PROJECT
WHEREAS, in September of 2005, the City awarded the
construction contract for the City Hall Project; and
WHEREAS, unforeseen circumstances may arise from time to time
that will necessitate minor adjustments and changes in the original construction
plan; and
WHEREAS, there are three items that we currently have prices for
and associated change orders have been submitted; and
WHEREAS, Change Order No. 4, ASI-14 and ASI-20, represents a -~
credit back to the City in the amount of $1,193.00 far the deletion of ACP and
gypsum board bulkheads in the basement and simplification of window re-
• installation; and
WHEREAS, Change Order No. 5, ASI-8, is for Millwork
adjustments and represents a net change in the amount of $1,997.00; and
WHEREAS, Change Order No. 6, .ASI-1 and ASI-13, is for
removing the steel decking and concrete mezzanine and installing a steel beam
for support in the amount of $8,522.00; and
WHEREAS, it is recommended that funds to finance change order
No. 4 and No. 6 be derived from the construction contingency and the funds for
change order No. 5 be derived from the Special Project Account for the City Hall
Project (account: 280-4180-7050-100); and
WHEREAS, Staff recommends that the City proceed with Option D
to address the HVAC Control issue; and
WHEREAS, it is recommended that the necessary funds to address
the HVAC Control issue be derived from the Special Project Account for the City
Hall Project (account: 280-4180-7050-100); and
WHEREAS, it is recommended that the twelve heating coils be
replaced; and
• WHEREAS, it is recommended that the necessary funds to replace
said heating coils be derived from the Special Project Account for the City Hall
Project (account: 280-4180-7050-100).
WHEREAS, it is recommended that the furniture contract be
adjusted to address a reassign of office space.
WHEREAS, the cost of this furniture contract adjustment is
$2,104.66 under State bid with funds to be derived from the Special Project
Account for the City Hall Project (account; 280-4180-7050-100).
NOW, THEREFORE, BE IT RESOLVED by the City Council of the
City of Mounds View, Ramsey County, Minnesota as follows:
1. That Change Order No. 4, 5, and 6 for the City Hall Rehabilitation
Project are hereby approved.
2. That Change Order No. 4 and 6 will be financed with funds in the
project contingency account.
3. That Change Order No. 5 will be financed with funds from the Special
Project Account for the City Hall Project (account: 280-4180-7050-
• 100).
4. Staff is authorized to direct the contractor to initiate Option D (Change
control system to DDC and Replace RTU No. 6) and to initiate the
replacement of the heating coils.
5. The furniture contract awarded on February 13, 2006 is hereby
amended to be increase by $2,104.66 with funds to be derived from
the Special Project Account for the City Hall Project (account: 280-
4180-7050-100).
