In granting a conditional use or special permit,the Planning and Zoning Commission or Board of Zoning <br /> Appeals,as applicable,may impose conditions to the extent the body concludes that such conditions are <br /> necessary to minimize any adverse effect of the proposed tower on adjoining properties. <br /> 5. Any information of an engineering nature that.the applicant submits whether civil,mechanical,or electrical, <br /> shall be certified by a licensed professional engineer. <br /> 6. An applicant shall submit the information described in this Section and a non-refundable fee as established <br /> by the City's annual fee ordinance to reimburse the City for the costs of reviewing the applications. <br /> B. Towers. <br /> 1. Information required. In addition to any information required for conditional use or special permit <br /> applications under the Zoning Ordinance applicants for a conditional use or special permit for a tower shall <br /> submit the following information: <br /> a. Complete application form. <br /> b. A scaled and dimensioned site plan(not less than one inch equals 50 feet)clearly indicating the <br /> locations,type and height of the proposed tower, on-site land uses and zoning,adjacent land uses <br /> and zoning(including when adjacent to other municipalities),adjacent roadways,proposed means <br /> of access, setbacks from property lines,elevation drawings of the proposed tower and any other <br /> stnuctures,topography,parking and other information necessary to assess compliance with this <br /> ordinance. <br /> C. Legal description of the parent tract and leased parcel(if applicable). <br /> ® d. The setback distance between the proposed tower and the nearest residential property. <br /> e. The separation distance from other towers described in the inventory of existing sites submitted <br /> pursuant to Section V.B. 1. shall be shown on a map. The applicant shall also identify the type of <br /> construction of the existing towers(s)and the owners/operator of the existing tower(s),if know. <br /> f. A landscape plan showing specific landscape materials,locations and installations sizes. <br /> g. Method of fencing,if any, including height,material,style, and color and,if applicable;the method <br /> of camouflage and illuminations. <br /> h. A description of compliance with Sections V.A.,V.B. 1-17,VIII(b) (4),and all applicable federal, <br /> state or local laws. <br /> i. A notarized statement by the applicant as to whether construction of the tower will accommodate <br /> collocation of additional antennas for future users. <br /> j. Identification of the entities providing the backhaul network for the tower(s)described in the <br /> application and other wireless telecommunications sites owned or operated by the applicant in the <br /> municipality. <br /> k. A description of the suitability of the use of existing towers,other structures or alternative <br /> technology not requiring the use of towers or structures to provide the services to be provided <br /> through the use of the proposed new tower. <br /> 2. Factors Considered in Granting Conditional Use or Special Permits for Towers. In additional to any <br /> standards for consideration of conditional use or special permit applications pursuant to the Dublin Zoning <br /> Ordinance,the Planning and Zoning Commission or Board of Zoning Appeals shall consider the following <br /> factors in determining whether to application should be approved.
