ANTHONY <br /> BID PROPOSAL FOR HANDICAP SIDEWALKS , ETC. <br /> Bid to be Opened and Awarded August 31 , 1983 <br /> Purpose: This proposal concerns the upgrading of sidewalk approaches <br /> to street level . <br /> Prices : . Prices subject to change will be rejected. <br /> Bond: A bond equal to the. cost of the project is required, <br /> conditioned as to the quality of material, workmanship, .-and <br /> completion on time. <br /> Insurance : Proof of adequate liability .and workman's compensation <br /> must be submitted to the City of St. Antyony shortly <br /> after the award form. Quotes shall be submitted on <br /> this form. <br /> Area : The areas involved are ten approaches on St. Anthony <br /> Boulevard from Roosevelt to Silver Lake Road and one <br /> located at Kenzie Terrace and Lowry. Also to be included <br /> is a driveway curb at the southwest approach to the City <br /> Hall parking lot. <br /> • _-Extras : Any, additional work the City,.of.- St-.-Anthony may decide - -to have done will be decided on an individual basis and <br /> prices would be negotiated at that time. <br /> Items : <br /> ( 1) Concrete shall be 3A32 or better. <br /> (_2) All curbing affecting the sidewalk approach shall be <br /> removed to the nearest joint, and repoured with an adequate <br /> opening for the sidewalk involved with a bevelled edge <br /> on each side of the opening. <br /> (3) Enough sidewalk shall be removed to the nearest joint or <br /> to a sawed joint to provide for a wheelchair slope , not <br /> to exceed 10 degrees . <br /> ( 4) Contractor shall replace blacktop as needed to return <br /> road surface to original condition, and repair dirt and <br /> sod areas involved. <br /> (5) All work shall be completed by October 15 , 1983. <br /> (6) A penalty of $100 per day will be assessed for each <br /> working day beyond October 15 , 1983. <br />
