Employees who terminate and are rehired in the same plan <br /> year may not file new elections for the balance of that plan <br /> year. <br /> Termination <br /> The Premium Conversion Plan has been designed to comply with <br /> all current laws regarding cafeteria benefit plans. The plan, <br /> however, may be changed, amended, or terminated at any time. <br /> Your participation in the .plan will cease upon termination <br /> of your employment, if you no longer meet the eligibility <br /> requirements, or if the plan terminates. <br /> If you are enrolled in the group medical plan when your <br /> participation ends, you may be able to continue your benefits, <br /> however, contributions will no longer be able to be made through <br /> salary reduction. <br /> The Summary Plan Description is merely attempting to explain <br /> the benefits provided pursuant to the Plan, and is not the <br /> legally controlling document.
