PART IV SUBSIDY PROGRAM <br /> (1) How long has the applicant.'s subsidy program been in existence? Just beginning <br /> • or, when is it to become effective? As soon as possible <br /> . (2) Clearly and completely describe the subsidy -program. Organize your answer in the manner - <br /> most appropriate. Please include the following areas. <br /> (a) Nkat persons are eligible and what trees are eligible for the local subsidy? <br /> (b) Method for determining amount of subsidy? <br /> (c) Records maintained, accounting methods used, and paper work involved? r <br /> (d) - Procedure or arrangements made for rF=ving diseased trees from private residential <br /> property. <br /> APPENDIX E: Please include copies of forms, records or other documents which will <br /> support the applicant's explanation of the administrative framework of <br /> the local subsidy program. <br /> a.. . A11_ owners of private residential property and all species of <br /> elms and oaks . <br /> b. The subsidy will be the lessor .of $100.00 or 50% of the cost of t <br /> removing the . tree. <br /> c. As follows: <br /> i <br /> 1. Record of private residential proprty owners who removetrees <br /> and request subsidy. <br /> • 2. Number of trees removed from each pro erty. <br /> 3 . Certification that trees removed a uirements of the State <br /> of Minnesota. <br /> 4 . Record of subsidy paid by city. <br /> 5. All other records required .by State of -Minnesota or to comply j <br /> with accepted . accounting procedures. i <br /> 6 . City Treasurer will account for all fiscal transactions <br /> using acceptable accounting procedures similar to accounting <br /> procedures used for all city fiscal transactions . <br /> 7 .
