• STATE OF MINNESOTA <br /> Minnesota Pollution Control Agency Amount: $38,252.60 <br /> City of St. Anthony <br /> 3301 Silver Lake Road <br /> St. Anthony, Minnesota 55418 <br /> AMENDMENT NO. 3 TO CONI?,A= NO 32300-19754 <br /> WHEREAS, the state of Minnesota acting through its Pollution Control <br /> Agency (State) has a contract identified as Contract No. 32300-19754 <br /> (Contract) with the city of St. Anthony (City) to rehabilitate city well #3, <br /> obtain pipeline easements, and provide operation and maintenance for the <br /> Granular Activated Carbon (GAC) Water Filtration Plant, and <br /> • WHEREAS, paragraph 4.1 of the Contract provides that: <br /> The City shall furnish the necessary personnel, materials, services, land, <br /> and other facilities to perform its responsibilities under this contract <br /> for the portions of the project described in this part and for its <br /> 0 and M [operations and maintenance] according to the O and M Plan. . . . <br /> WHEREAS, paragraph 4.4 of the Contract provides that: <br /> The date certified in the Interim RA [Remedial Action] Report that the <br /> construction is complete and the remedy is operational and functional is <br /> the date when the Extended RA cram ences. The City shall operate and <br /> maintain the GAC facility and associated parts of the system as directed <br /> by the 0 and M Plan, in accordance with Parts 6.0 and 7.0 of the <br /> contract. . . . <br /> WHEREAS, on February 28, 1991, Camp Dresser and McKee, Inc. as the State's <br /> Consultant Engineer certified that constriction of the GAC facility was <br /> complete, and <br /> • <br />
