THIRD PARTY AGREEMENT <br /> URBAN HENNEPIN COUNTY <br /> COMMUNITY DEVELOPMENT BLOCK GRANT PROGRAM <br /> EXHIBIT 1 <br /> The following activity shall be carried out by the Provider under the terms of <br /> this Agreement and the details and processes set forth below: <br /> 1. ACTIVITY: Senior Center-Operations <br /> 2. LOCATION: ADDRESS: Citywide <br /> CENSUS TRACT: <br /> 3. NUMBER: 126 <br /> 4. BUDGET: $5,608 <br /> 5. BENEFIT: L/M (Limited Clientele) <br /> 6. DESCRIPTION: Pay for a portion of the Senior Program Coordinator's salary. <br /> This project allows for the continuation of the following activities: day <br /> trips and tours, recreational and educational programs and activities, <br /> youth referral for home chore assistance, and referral for specific social <br /> service needs. <br /> 7.
