CONFINED SPACE ENTRY PROCEDURES <br /> • Class 1A -- Entry Procedure <br /> 1. Annual permit issued. <br /> 2. Test monthly and record readings from monitor. <br /> Class 1B -- Entry Procedure <br /> 1. Test atmosphere before entry for 5 minutes. <br /> 2. Record atmosphere reading on C.S.E. permit. <br /> 3. Use protective equipment required and check off on entry permit. <br /> 4. Wear monitor at all times while in confined space. <br /> 5. Continue ventilation all the time while person is in confined space. <br /> 6. Confined space entry permit required prior to entry. <br /> Class II -- Entry Procedure <br /> 1. At least one person stand by outside of space prepared to give assistance <br /> in case of an emergency. <br /> 2. Voice or visual communication shall be maintained between standby person <br /> and person in the confined space. <br /> 3. Standby person shall not enter the confined space. If person in confined <br /> space is overcome, standby person shall alert fire department. <br /> 4. Test atmosphere before entry for 5 minutes. <br /> 5. Record atmosphere reading on C.S.E. permit. <br /> 6. Approved safety belt on harness with attached line should be used where <br /> • practical or feasible. <br /> 7. Use proper protective equipment and check off on C.S.E. permit. <br /> 8. Person entering should wear monitor and continue monitoring while in <br /> confined space. <br /> 9. Confined space entry permit required. <br /> Class III -- Entry Procedure - Known Hazard Exist <br /> 1. We will not enter confined space until hazard is clear. <br /> RESCUE PROCEDURE <br /> A. Standby person is not to enter confined space <br /> 1. Request assistance immediately, call Fire Department. <br /> 2. If person is attached to life line and hoist in a confined space, try to <br /> rescue from outside. <br /> 3. Do not enter the space to attempt rescue. <br /> CITY OF ST.
