REQUEST FOR COUNCIL CONSIDERATION <br />Meeting Date: July 23, 2019 <br />Resolution-Approving the 2019 Agreement for a Community Case Manager with Northeast Youth <br />and Family Services (NYFS) <br />OVERVIEW: <br />In front of you this evening is a resolution to approve a service agreement with Northeast Youth & <br />Family Services to provide a Community Case Manager. <br />If approved, Northeast Youth & Family Services (NYFS) would hire a community case manager to <br />serve our Police Department/community and our partner Law Enforcement (LE) agencies in <br />performance of functions related to connecting referred households/individuals with basic services <br />related to mental health or any other basic family need. The Community Case Manager would provide <br />“follow up” activities to referred households which will include efforts to establish a relationship, <br />assess needs, develop a Goal Action Plan, connect to resources, and follow up on the impact of the <br />Goal Action Plan. The case manager will work with the Police Department to identify <br />individuals/households that would be appropriate candidates. We do have several <br />individuals/households that would be candidates. The anticipated end result would be a decrease in <br />multiple calls for service by LE (possible fire rescue) on sometimes a daily, weekly and monthly basis. <br />As you are aware, these types of calls for service can be very involved and exhaust city resources. <br />Unfortunately, and sometimes tragically, some “person in crises” calls can turn into unavoidable and <br />unwanted use of force encounters. <br />This initiative will serve as a considerable action step toward meeting the city and police departments’ <br />goals of participating in public safety joint mental health initiatives as referenced on the city’s strategic <br />pyramid. <br />Highlights of the attached agreement include: <br />•$10,000 annual cost <br />•Initial term is one year and may be renewed or extended <br />•Either party may terminate this Agreement, without or without cause, by providing 60 days <br />written notice to the other party. <br />53
