Description of the Request (or a separate detailed narrative explaining the project) <br />Location: 2924 Anthony LN Saint Anthony, MN 55418 <br />Overview: Serenity Adult Daycare will serve individuals ages 18 years and older who have disabilities and or special needs. <br />We will provide temporary relief for families from the <br />day-to-day responsibilities of supporting relatives with disabilities. Serenity Adult Daycare will provide a variety of social, <br />interactive, and recreational support services in a supervised environment. We will provide services for those at our day <br />program for 6 hours each day from 9am-3pm. <br />On-site Activities: On-site activities will include crafts, seasonal activities like reading, holiday parties, DIY projects, card & and <br />board games, etc. Also, for our clients who <br />are interested and capable, we will offer job aid services that help these individuals get jobs <br />in the community by assisting them with their resumes, job applications, skill development, etc. <br />Off-site Activities: Off-site activities will include buying supplies at Target, Walmart, and going to the dollar store, etc. <br />Number of Business Vehicles: Currently we do not have any company vehicles at this location but have a plan to make space <br />for 2 or 3 of such vehicles. They will be vans that carry between 15-20 passengers. <br />Number of Adults being served each day: Currently there are no adults being served at this location, but we plan on providing <br />services to up to 100 adults. <br />Number of employees: Currently we have no Adult Day employees. Depending on how many clients come to the day program <br />in the future we will have to provide 1 employee for every 10 clients. <br />Drop Off & Pick Ups: The parking lot will have sufficient parking space for our day-to-day operations. How the process will <br />work is that each of our employees will be assigned to pick up certain clients on their way to work and take them home on their <br />way home from work. The employee simply parks and helps those who need help get out of the vehicle and safely to and from <br />the day program. For those who are less mobile, we will have disability placards where they can park closer to the building. <br />Employees arrive at our day program around 9 a.m. and take the clients home at 3 p.m. <br />Adjacent Businesses: The property is relatively secluded and will not disrupt adjacent businesses. <br />Potential Impacts: We shouldn't have much impact on neighboring businesses as our day-to-day operations will be either out <br />in the community or back at our day program <br />Attachments <br />Building Floorplans.pdf <br />Acknowledgement and Signature <br />I acknowledge that I have read all of the information listed in the City of St. Anthony Village Land Use Application and fully <br />understand that I am responsible for all costs incurred by the City related to the processing of this application. If additional fees are <br />required to cover costs incurred from processing of the application, the City has the right to require additional payment from one or <br />more of the undersigned, who shall be jointly liable for such fees. Such expenses may include (but are not limited to) direct city <br />payroll and overhead costs, fees paid to consultants and other professionals, and the cost of printing, mailing, and supplies. <br />Applicants are advised that an escrow deposit is required at the time of the submittal of the land use application to offset costs <br />associated with the proposed project. Unused portions of an escrow are returned to the applicant upon successful implementation of <br />an approved plan.
